The Health Is Home project seeks to strengthen community health systems by revitalizing an under-resourced rural clinic through medical equipment donations, health awareness, and public engagement. This initiative integrates immediate support (free consultations and supplies) with long-term community empowerment through education on disease prevention and substance abuse. The project will operate in five phases and is aligned with the Sustainable Development Goals (SDGs), particularly SDG 3 (Good Health), SDG 6 (Clean Water and Sanitation), and SDG 16 (Strong Institutions).

Project Objectives
Build a scalable, transparent model for rural healthcare intervention.
Equip a selected clinic with essential medical tools (e.g., malaria kits, thermometers, gloves, weighing scales).
Conduct a Community Health Outreach Day that reaches at least 250 people with free consultations and drug abuse awareness.
Reduce misinformation and stigma surrounding drug addiction in rural communities.
Project Phases
Phase 1: Mobilization & Clinic Nomination (July – August 2025)
Goal: Engage the public in nominating deserving clinics
Activities:
- Design and launch #NominateAClinic campaign
- Deploy Google Forms and paper-based nomination tools
- Score entries via selection committee using need-based criteria
- Public announcement of selected clinic
Phase 2: Fundraising & Partnerships (July – October 2025)
Goal: Raise ₦3,000,000 from diverse sources
Activities:
- Launch digital micro-giving campaign
- Create and manage crowdfunding platform
- Submit proposals to foundations and CSR leads
- Recognize donors with public shout-outs, branding exposure, and certificates
Phase 3: Procurement & Clinic Preparation (September – October 2025)
Goal: Equip the selected clinic and train outreach volunteers
Activities:
- On-site needs assessment
- Purchase and deliver medical supplies
- Provide clinic signage and minor sanitation upgrades
- Train 10–15 volunteers on outreach procedures
Phase 4: Health Outreach Implementation (November 2025)
Goal: Deliver free health services and raise awareness
Activities:
- Host Health Outreach Day with screening, counseling, drug awareness, and hygiene demos
- Use local talent for storytelling, spoken word, and drama
- Distribute hygiene kits and educational materials
Phase 5: Monitoring, Reporting & Sustainability (Dec 2025 – Nov 2026)
Goal: Measure impact and document outcomes
Activities:
- Monthly feedback from clinic staff
- Video and testimonial collection from patients and volunteers
- Final impact report and short documentary
- Community feedback roundtable
