The Health is Home project by Green Network Initiative hopes to revitalize under-resourced rural clinics by providing essential medical supplies and hosting a free Community Health Outreach Day. We’re calling on you, community leaders, health professionals, and concerned citizens, to help us identify clinics that need support.
✅ What Are We Looking For?
We aim to support clinics that:
- Serve rural or underserved populations
- Have limited access to basic medical tools and drugs
- Are open to community collaboration
- Can provide a local contact or liaison
📩 How to Nominate
Fill out our simple nomination form to tell us about a clinic in need. You’ll be asked for:
- Clinic name and location
- Type of services currently offered
- Challenges or needs (e.g., equipment, staffing, outreach)
- A point of contact at the clinic

In many rural communities, health clinics are the first and sometimes only line of care, yet they often lack the basic supplies to serve their people effectively. Through Health is Home, we don’t just donate items; we help clinics regain their purpose as trusted centers for care, dignity, and hope.
By nominating a clinic, you’re enabling us to:
- 🩺 Provide life-saving tools like malaria test kits, beds, weighing scales for children and expectant mothers, and more.
- 📚 Host a free Health Outreach Day, offering blood pressure checks, health education, basic screening, and drug abuse awareness talks—all with volunteer nurses, doctors, and community leaders.
- 🤝 Engage local champions, who will be trained to maintain the impact, build trust, and connect residents with follow-up care.
